Practical Information
Our main location is at Valeriusplein 40.
The branch where you can also make appointments is at Olympiaplein 142, next to the Stadionapotheek.
When are we open?
Opening hours: We are open from 08:00 to 17:00. The practice is open all year round.
The practice can be reached by telephone between 08:00 and 17:00.
And between 08:00-11:00 and 13:00-14:30, a live chat with the GPs is available. You can chat live with the GP within 5 minutes. You can find this via our website (homepage IA).
The practice uses a web app via our website, making us available 24/7 for repeat prescriptions. If you call us, we have a callback system. You can also leave prescriptions or a message by phone.
On Fridays, we have an evening consultation hour (by appointment only and at the location Valeriusplein 40).
Urine
Do you think you have a urinary tract infection?
1: You can drop off your morning urine sample at the front desk at Valeriusplein 40 during the following times: in the morning between 10:00 AM and 12:00 PM; in the afternoon between 1:00 PM and 4:00 PM.
2: You can fill out this form in advance and send it via e-consult (this will then be in your file, so the assistant won’t need to ask you for privacy-sensitive information at the front desk).
Laboratory
You can visit our practice for laboratory tests (blood draws and urine tests) every weekday except Thursday between 8:00 AM and 10:45 AM.
Urine samples can be submitted in the designated containers until 4:00 PM. If submitted in other containers or bags, the test results will not be reliable and we will not be able to process them.
Please note: for some tests, you must have an empty stomach.

Requesting a second opinion and specialized lab tests
We regularly receive requests for diagnostic testing on behalf of a foreign doctor or specialist. For example, IVF treatment abroad. Primary care providers do not have the capability to test for these specific values. Results requested by us can be found in your portal; you can share these with your specialist yourself.
We refer you to the laboratories that can perform these tests for you: (at your own expense)

We cannot interpret the results of any tests requested through us but intended for a foreign physician. This is done by the specialist who also requests the tests.
Second opinion: these requests cannot be made by the primary care physician. Your treating physician (the specialist with whom you are registered for this condition) can only refer you for a second opinion. You must always discuss this with your treating physician. The primary care physician cannot assist with this.
Ultrasound
Dr. Jongebreur and Dr. Hennipman are certified sonographers. They can perform ultrasounds of the abdomen, skin, muscles, and joints.

So Yourself An advance directive is a document that states your wishes regarding treatment or end-of-life care. This directive is used when you are no longer able to make decisions yourself.
Advance Directives: An Example
A living will does not have to be a lengthy or complicated text. It is important, however, that you write the will in your own words. You should then discuss with your primary care physician whether your wishes are feasible and practical. This ensures that the will is truly yours and suits your needs. Your living will will be included in your medical record.
Below are a few sample questions you can answer in your written advance directive:
- Whether you would like to be resuscitated
- Whether you have ever thought about which treatments you do or do not want to receive
- Whether you would like to go to the ICU at the end of your life
- If you have a wish for euthanasia. What constitutes unbearable suffering for you, and why is that the case?
- Who is your (legal) representative: who is authorized to make medical decisions on your behalf if you are no longer able to do so yourself? What is your representative’s name, address, and phone number?

Changed your mind?
You may change your mind later or have doubts about your decision. It is important to discuss this with your family and your primary care physician. The physician will then update the records in your medical file.

Shingles Vaccine

To safeguard the independence of our general practitioners, we do not perform medical examinations for sports, sports medical examinations, travel, study, or work.
In doing so, our general practitioners act in accordance with the rules established by the KNMG, the professional organization for general practitioners. You can make an appointment for this at Nauwkeurig.nl; they hold a weekly consultation hour by appointment at MC Zwaansvliet, in our general practice. You can make an appointment by telephone. For the retrieval of medical information by a third party—for example, an insurance company, permit, or legal purpose—the requesting party may submit a request to us through legally valid channels. You must authorize them to do so.
