Patient portal
The patient portal is a modern addition that we offer you as a practice. The application is available in any web browser, whether you use a PC, laptop, tablet, or smartphone.
By logging into the application, you as a patient can immediately arrange various matters with your general practice without having to wait on the phone. For example, you can schedule an appointment online in designated time slots, order repeat prescriptions, view blood and X-ray results including the doctor’s advice, and send an email consultation to your GP yourself. In this way, as a practice we also meet the national requirement for electronic access to your own patient record.
Medical information is kept secure because the portal is developed according to standard practices for handling sensitive information. Secure network systems and customized software are used.
How the Patient Portal Works
Below you can read which steps are necessary to create a profile. Due to security requirements, it is important that each person has their own mobile number and their own email address. This information CANNOT be used for multiple profiles.
Create by GP's practice
A patient portal must be created by your GP practice using your personal data. To comply with all privacy legislation, it is important that only you can access your medical records.
Requirements
- The email address must be a personal email address used only by you!
- The mobile phone number must be a personal number used only by you.
Contact us
Please fill in the form below with your personal details. We will create your account within 72 hours, and you will receive an email from us to validate your details.
Validation
You will receive an email requesting you to click on a link. After this, you will receive a confirmation code via SMS to the mobile number you provided. In this way, the system verifies whether the details are correct. Subsequently, you can choose your own password.
Login
Using the details you just obtained (username and password), you can now log in via the patient portal. You will receive an SMS verification again. This is a legal requirement as we work with medical data.
Request account
Please note: Only for already registered patients over the age of 16.
Please note: You must use a personal email address for each person.
Please note: A data verification will be performed by the practice before you can proceed.
